Secondary Insurance Information

MLC has provided athletes with an insurance form and an explanation of insurance provided by the institution only in addition to the students' insurance coverage. If your child was recently injured while participating in an intercollegiate athletic event at Midland Lutheran College. Midland does have an excess insurance policy, which may cover the remaining medical charges not paid by the student’s primary insurance company. All information about the injury and insurance procedures will be mailed to you, please hang on to the information!!

Items that are needed for the College to file insurance claims on behalf of your child:
• Injury Claim Form – this form is filled out by the athlete and Athletic Trainers at the college.
• Itemized Claim Form– an itemized bill is needed for every visit from every doctor/hospital that treats your child. Doctor’s offices need to submit an HCFA-1500 form, and hospitals need to submit a UB-92 form, along with an Explanation of Benefits (EOB) from your primary insurance carrier. If your child needs to purchase a brace for rehabilitation purposes, these can also be submitted to SUMMIT with an HCFA-1500 form. Once your primary insurance has paid their portion of the charges, please notify the doctor/hospital and have them send an itemized bill (HCFA-1500, UB-92) and EOB directly to Midland’s secondary insurance company:

Many doctors/hospitals are not aware of this coverage offered by Midland, so it is your responsibility to inform them to send the itemized bills and your EOB from your insurance to NAHGA.

Once the claim has been sent to NAHGA Claim Services, it usually takes between 4 – 6 weeks for NAHGA to process each claim. If you continue to receive a statement each month from a doctor/hospital, chances are the information needed was never submitted to NAHGA for processing. You can check the status of your claim by calling NAHGA at (800) 952-4320.
